Parent's Consent on usage of student's photographs and AV's
I, hereby consent that any of my child's photograph/audio-visual/artwork/coursework created within Mun Lun School's programs, shall be the property of Mun Lun School and can be used by the school and its employees and agents in any manner in connection with its regular business purposes. 我仅此同意，本人之小孩在明伦学校就读校期间的有关照片，视听材料，书画稿及课程作业，均属于明伦学校所有；明伦学校及其雇员或代理人有权以任何方式使用这些材料。

Mun Lun School Release and Waiver of Liability
We sign this Release individually and on behalf of our child, in consideration for Mun Lun School allowing our child to participate in School-related trips, excursions, and physical or other activities. We assume the risk of personal injury or property damage arising out of our child’s participation such as trips, excursions, and physical or other activities. We acknowledge and understand that any School-related trip, excursions, and physical or other activity has certain inherent risks, including those related to transportation, the use of certain facilities or equipment and unforeseen hazards or conditions.
我们代表我们的孩子以及我们个人签署这份协议书，同意我们的孩子参加学校集体出游，体育及其他所有与学校相关活动。我们将承担孩子在参加旅行，出游，体育和其他活动时发生的个人伤害和财产损失的风险。我们承认和理解任何学校相关的活动都存在固有的风险。这些风险还包括交通方面的风险，使用设备用具的潜在危险，以及其他不可预见的危险和状况。
We, individually and on behalf of our child, hereby release Mun Lun School, its Board of Directors, School, Principal, faculty, staff, employees, insurers, agents, representatives, volunteers and chaperones from all liabilities, claims, actions and causes of action, for any injuries, losses, death, damage or costs (including reasonable attorneys’ and experts’ fee) asserted against or incurred or sustained by Mun Lun School arising out of or relating to our child’s attendance at or participation in any School-related trip, excursions, and physical or other activity, or our child’s use of any facilities or equipments, whether owned by or under the control of the School or otherwise.

We understand that while participating in any School-related trip, excursions, and physical or other activity, our child must comply with all instructions and directions provided by the personnel of Mun Lun School. We shall notify Mun Lun School in writing if our child has any medical or physical ailment, disability or other condition which affects our child’s ability to participate fully in any School-related trips, excursions, and physical or other activities.
我们理解我们的孩子在参加任何学校相关的旅行，出游，体育和其他活动时必须遵守所有明伦学校人员提供的说明和指示。如果我们的孩子有任何身体疾病，残疾或其他可能会影响或限制我们的孩子参加任何学校相关的旅行，出游，体育或其他活动的情况，我们必须书面通知明伦学校。
